Why repo.shapeInstanceCounts refused with SHAPE_COUNTS_UNAVAILABLE: the
repo is above the live per-Shape size band, or the bounded read timed out.
A request-level refusal only, never a commit diagnostic. The vocabulary is
open: treat an unknown reason as “unavailable, retry later”, and
read totals from repo.stats.
